Individual Residential Services and Support

At A Mindful Care, we believe everyone deserves a place to call home where they feel safe, respected, and supported in living life on their own terms.

Our Individual Residential Services and Supports (IRSS) are designed to help adults with intellectual and developmental disabilities build independence while receiving individualized support in their daily lives. Depending on each person's needs and authorized services, residential support may be provided in a Host Home, a staffed residential setting (formerly known as PCA), or the individual's own home.

Support may include assistance with daily living skills, personal care, household routines, appointments, community involvement, and developing greater independence.

We work alongside individuals, families, guardians, and case managers to support personal goals, encourage choice, and create a living environment where each person feels valued and included.

Supported Community Connections

At A Mindful Care, we believe being part of a community means more than simply getting out of the house. It's about building friendships, discovering interests, learning new skills, and feeling a genuine sense of belonging.

Our Supported Community Connections program helps individuals with intellectual and developmental disabilities become more involved in their communities through meaningful, person-centered activities.

Activities may include recreation center programs, fitness and wellness opportunities, arts and crafts, community events, volunteering, and other experiences based on each person's interests and goals.

We focus on building confidence, strengthening social and independent living skills, encouraging personal choice, and creating opportunities for lasting community connections.

Our goal is to help individuals participate in activities they enjoy while developing skills and relationships that enrich their everyday lives.

Respite

Caring for someone you love is meaningful, but every caregiver deserves time to rest, recharge, and attend to personal responsibilities.

A Mindful Care provides short-term respite support for children and adults with intellectual and developmental disabilities, giving primary caregivers temporary relief while their loved ones continue receiving attentive, individualized care.

Depending on the individual's needs and authorized services, respite may include assistance with daily routines, personal care, recreational activities, and community participation.

Our goal is to provide a supportive experience for the individual while giving families peace of mind during their time away.

4

Mentorship

Everyone deserves to have a voice in the decisions that affect their life.

A Mindful Care provides mentorship services that help individuals with intellectual and developmental disabilities strengthen self-advocacy, build confidence, and develop a better understanding of their personal choices and responsibilities.

Through guidance, education, encouragement, and real-life learning opportunities, individuals can work on communicating their needs, understanding important health and safety information, navigating everyday challenges, and participating more actively in decisions about their lives.

Our approach focuses on helping each person recognize their abilities, develop greater independence, and feel empowered to speak up for themselves.

5

Community Connector

Every child deserves opportunities to explore their community, make friends, and participate in activities alongside their peers.

A Mindful Care LLC provides individualized Community Connector services for children with intellectual and developmental disabilities through Colorado's Children's Extensive Support (CES) waiver.

Through one-on-one support, children are encouraged to participate in age-appropriate community activities, discover new interests, strengthen social skills, and develop meaningful relationships.

Activities are guided by each child's interests, abilities, and individual goals, with an emphasis on inclusion, confidence, independence, and natural community connections.

Our goal is to help children experience the joy of belonging while building skills that support their continued growth.
